This Website Privacy Policy applies to Lifetime Dental (“we,” “us,” or “our”) and our website and any pages we own or control (collectively, the “Site”).
Important HIPAA Note: This Website Privacy Policy explains how we collect and use personal information through our Site (such as contact forms, online scheduling, payments, and cookies). It is not the HIPAA Notice of Privacy Practices (NPP), which governs how protected health information (PHI) is used and disclosed in clinical care. Our HIPAA NPP is available upon request from our office.

You may opt out of marketing emails using the unsubscribe link provided in those messages. Transactional or administrative messages may still be sent. Where required by law, you may also have rights to access, correct, or request deletion of certain personal information.
